PLEASE NOTE: We encourage you to register online before October 19 to ensure that a space is held for you. If you do not wish to use a credit card to register on the secure site, you may fax or mail a copy of your completed registration form accompanied by a check, credit card information or purchase order.

2.                Register Via Fax or Mail: Please fill out the fax form and fax it to 813-905-5801.

3.                Register Via Telephone: Call Small Business Development Center at USF 813-905-5806. Office hours are Monday through Friday, 9:00 am to 4:00 pm Eastern Standard Time.

Payments by credit cards (preferred): Course registration fee payment is preferred with a VISA or MasterCard via the following website:

All payments must be in U.S. dollars drawn on U.S. banks at the time of registration.

Payments by purchase orders: Fees to be paid by employers are to be verified by approved/numbered purchase orders. Purchase orders should include the name and address of the participant(s) and be sent with registration forms. (USF Federal I.D. - #59-3102112)

Receipts/confirmations will be mailed to pre-registered participants within one week after USF receives registrations.  Registrations without payments will not be processed.  Please contact Small Business Development Center @ 813-905-5806 or email kmanning@coba.usf.edu if you have questions about registration.

Cancellations: All cancellations must be received in writing and postmarked prior to the dates shown:

The University reserves the right to revise program content and presentations, or to cancel the program if registration criteria are not met or when conditions beyond USF's control prevail.  If the program is not held, the University's liability is limited to a refund of the registration fee paid by each participant.

Photographs and other recordings may be made for promotional purposes. If you do not wish to be included in any recordings, please inform the program staff when you check-in.
